Instructions For Filling Indian Passport Application Form

  Mandatory Documents:
 

Below documents are required to be submitted along with all applications, further documentation has been advised as per individual scenarios. Please read the below advise carefully to ensure correct compilation of the documents required.

  • Online Passport Application
  • 4 (four) recent photographs (2 inch X 2 inch) identical photographs showing frontal view of full face. Photographs should be in colour with white/off white background.
  • Printed & signed copy of the declaration form (to be attached with each application)
  • Annexure E – to be added with all the applications
  Re-issue of Passport on expiry or up to one year before expiry (having a valid Visa):
 
  • The current or recently expired Indian passport along with the self-attested photocopies of the Bio Page (photo page), last page with the address & observation page, if any observations have been noted, or any other visa page with any observation details should also be provided.
  • Residence Permit (card) wherever applicable needs.to be enclosed along with self-attested photocopies
  • If the applicant is married and his/her spouse name is not entered in the passport, he/she should submit a self-attested photocopy of marriage certificate and self-attested photocopies of Passport of the applicant’s spouse (original Passport & marriage certificate to be submitted at the time of application) for adding spouse name in the new passport. Please refer to the Addition/Deletion of Spouse name.
  • This is a mandatory requirement; applicants who do not have their spouse’s name endorsed on their passport, should carry the above mentioned documents (self-attested photocopy of marriage certificate, self-attested photocopies of Passport of the applicant’s spouse, along with the original Passport & marriage certificate) when applying to renew their Indian Passport
  • For passports that expired for up to one year, a self-explanatory letter as to why the passport was not renewed in time needs to be submitted.
  Re-issue of Passport Expired for more than one year (having valid Visa):
 
  • The recently expired Passport having valid Visa or Residence Permit is required to be enclosed with self-attested photocopies of pages containing detailed passport particulars, Visa page and the page containing observation (s), if any. In case the latest Passport is additional booklet(s) issued to the original Passport, the additional booklet(s) plus the original Passport must be furnished.
  • Copy of the valid Visa or Residence Permit card with self-attested photocopies.
  • Applicant is required to submit an Affidavit in the prescribed format (Please click here for the application form).
  • ID proof carrying photograph of the applicant like Driving License, Ration Card, Electoral ID card, PAN Card, ADHAAR Card etc. (IDs should preferably be from India as UK issued IDs would be accepted only as secondary proof).
  • Personal Particular Form (in duplicate) for Police Verification (Please click here for the form, with one photograph) may be required to be filled in some cases where identity is not conclusively established.
  • If the passport is expired for more than one year but less than 3 years, a notarized affidavit as to why the passport was not renewed after expiry for such a long time needs to be submitted. If the passport is expired for more than 3 years, Personal Particular Form (in duplicate with 4 extra photos) is required to be attached with the application form in addition to a notarized affidavit as to why the passport was not renewed after expiry for such a long time.
  Addition/Deletion of Spouse’s name in the passport:
 
  • The current Passport having valid Visa or Residence Permit is required to be enclosed with photocopies of pages containing detailed passport particulars, Visa page and the page containing observation (s), if any (photocopies should be self-attested by the applicant). In case the latest Passport is additional booklet(s) issued to the original Passport, the additional booklet(s) plus the original Passport must be furnished.
  • Original marriage certificate with a photocopy and self-attested photocopies of Passports of the applicant’s spouse (original passport to be submitted at the time of submission of the application),
  • OR
    Divorcees applying for a change of name are required to furnish the divorce decree authenticated by the Court (if the divorce decree is issued by the local UK Court, it should be apostilled by the UK Foreign & Commonwealth Office),
  • OR
    In case of change of name following death of spouse, photocopy of death certificate of spouse (original to be shown at the time of submission the application),
  • OR
    Remarried applicants applying for change of name/change of spouse’s name will be required to produce the divorce decree/death certificate in respect of first spouse, marriage certificate from the Registrar of Marriages and photocopy of present spouse’s Passport (original passport to be shown at the time of submission of the application)
  • OR
    If the divorce decree or marriage certificate is issued by other authorities (country) other than UK authorities, it is to be authenticated by the Indian Mission/Post in that country or apostilled by the competent authority in that country (if that country is a member of Apostolic County and if a document apostilled by that country is entitled to recognition in India). If either of the spouse is in India and holding passport, self-attested photocopies of the passport and original marriage certificate is required to be submitted along with the application for addition of spouse’s name.
  Change of address in the passport:
 
  • The current Passport having valid Visa or Residence Permit is required to be enclosed with self-attested photocopies of pages containing detailed passport particulars, Visa page and the page containing observation (s), if any. In case the latest Passport is additional booklet(s) issued to the original Passport, the additional booklet(s) plus the original Passport must be furnished.
  • Documents establishing new address in the form of utility bills, latest monthly bank statements, etc.
  Change of signature:
 
  • The current Passport having valid Visa or Residence Permit is required to be enclosed along with self-attested photocopies of pages containing detailed passport particulars, Visa page and the page containing observation (s), if any. In case the latest Passport is additional booklet (s) issued to the original Passport, the additional booklet (s) plus the original Passport must be furnished.
  • Attested or Notarised Affidavit
  Change in appearance:
 
  • The current Passport having valid Visa or Residence Permit is required to be enclosed along with self-attested photocopies of pages containing detailed passport particulars, Visa page and the page containing observation (s), if any. In case the latest Passport is additional booklet(s) issued to the original Passport, the additional booklet(s) plus the original Passport must be furnished.
  • A Sworn Affidavit in the prescribed format (please click here for the form).
  • For Minor Applicants 'Change of Appearance' affidavit is not applicable for minor applicants renewing their passports that are Under 16. All minor applicants should have:
  • Documentation which prove that the local authorities have been informed of the substantial change in appearance (The applicant needs to submit Photo ID issued by local authorities of new appearance)
